LIST OF REQUIREMENTS

  1. Copy of Driver’s License
    If not available, a copy of your passport or permanent resident card
  2. Social Security Card Number
  3. Copy of Current Professional License/Certification
  4. Copy of Current CPR Certification
    Certification must be through the American Heart Association, or at least completed under the guidelines of the American Heart Association
  5. Copy of up-to-date automobile insurance card
  6. Copy of current physical examination
    Must be undergone within six (6) months from the date of hiring in order to be considered “current”
  7. Copy of Annual PPD Test
    If positive result:
    – Submit a copy of the PPD test with a positive result
    – Submit a copy of the x-ray with a negative result
  8. Copy of current professional liability insurance (applicable to independent contractors only)
