Our skilled nursing services are delivered by licensed nurses who bring hospital-level clinical expertise into the home. Every episode of care begins with a thorough assessment of the patient’s condition, medications, home environment and support system, so the plan of care reflects exactly what that person needs.
From complex wound management to disease-specific teaching, our nurses coordinate closely with the patient’s physician and family. The goal is always the same: keep the patient safe, comfortable and improving at home, and reduce avoidable trips back to the hospital.
What this service includes
- Initial comprehensive evaluation visit to the patient.
- Provide skilled nursing services that require more specialised nursing care, which include, but are not limited to:
- Wound care treatment
- Wound Vac
- Patient and caregiver teaching on disease management, but not limited to teaching about diabetes and insulin administration, congestive heart failure, hypertension, other cardiovascular diseases, and other body systems
- Respiratory equipment teaching and management
- Colostomy care
- Catheter care (Foley / Supra-Pubic Catheter)
- Injections and lab work (only in relation to skilled nursing needs)
- Medication Management
