From complex wound management to disease-specific teaching, our nurses coordinate closely with the patient’s physician and family. The goal is always the same: keep the patient safe, comfortable and improving at home, and reduce avoidable trips back to the hospital.

• Initial comprehensive evaluation visit to the patient.

• Wound care treatment
• Wound Vac
• Patient and caregiver teaching on disease management, but not limited to teaching about diabetes and insulin administration, congestive heart failure, hypertension, other cardiovascular diseases, and other body systems
• Respiratory equipment teaching and management
• Colostomy care
• Catheter care (Foley / Supra-Pubic Catheter)
• Injections and lab work (only in relation to skilled nursing needs)

Our home health aides provide the hands-on, everyday help that makes independent living possible. Working under the supervision of a nurse and within the physician-ordered plan of care, aides assist with the personal tasks that have become difficult or unsafe to manage alone.

Beyond the practical help, aides become a familiar, reassuring presence in the home. They notice changes in appetite, mobility and mood, and report them to the clinical team so small problems are addressed before they become serious ones.

Our physical therapists help patients recover strength, balance and confidence after surgery, injury, stroke or a period of decline. Therapy takes place in the patient’s own home, using their real stairs, doorways and furniture — which makes progress directly transferable to daily life.

Each programme starts with an evaluation of strength, range of motion, gait and fall risk, then progresses at a pace the patient can sustain. Families are taught how to assist safely so gains continue between visits.

Occupational therapy focuses on the tasks that fill an ordinary day — dressing, bathing, cooking, writing, getting in and out of a chair. Our therapists break those activities down, rebuild the underlying skills, and adapt the approach or the environment so the patient can do more for themselves.

Where ability cannot be fully restored, adaptive equipment fills the gap. Our therapists recommend and train patients on the tools and techniques that restore genuine independence rather than dependence on others.

Our speech-language pathologists treat difficulties with speech, language, voice, cognition and swallowing — problems that commonly follow a stroke, a neurological diagnosis or prolonged hospitalisation. Restoring communication restores a person’s ability to take part in their own care and their own family life.

Swallowing safety is equally central. Evaluation and treatment of dysphagia reduce the risk of aspiration and pneumonia, and give families clear, practical guidance on diet textures, positioning and pacing at mealtimes.

Illness is never only medical. Our medical social workers help patients and families work through the social, emotional and financial pressures that surround a serious diagnosis or a change in ability — from insurance and benefits questions to caregiver strain and long-term planning.

They also connect households to the wider network of community resources available to them. By assessing needs and coordinating those services, our social workers advocate for your needs and help you make informed decisions about your care.

A plan of care only works if the right equipment, medication and test results are in place at the right time. Assist on Call arranges the durable medical equipment a patient needs at home, along with the training to use it safely and confidently.

We also coordinate pharmaceutical and laboratory services so prescriptions and diagnostic results move quickly between the patient, the physician and the clinical team — without the family having to chase them.
