Health and medical information, including diagnoses, physician orders, plans of care, medications and clinical notes, where you are a patient or a person being referred for care.
Insurance and payer information necessary to verify coverage and bill for services.
Employment information such as résumés, licence and certification numbers, references and background-check results, where you apply to work with us.
Internet and device information such as IP address, browser type, pages viewed and referring pages, collected automatically when you use this website.
How we use your information
To assess needs, arrange and deliver care, and coordinate with your physicians and other providers.
To verify insurance coverage, obtain authorisations and bill for services rendered.
To respond to your enquiries, appointment requests and referrals.
To evaluate applications for employment and to meet licensing, screening and training requirements.
To operate, secure and improve this website.
To comply with legal, regulatory, accreditation and public-health obligations.
How we share your information
With physicians, hospitals, laboratories, pharmacies, equipment suppliers and other providers involved in your care.
With health plans, Medicare, Medi-Cal and other payers for coverage verification, authorisation and payment.
With service providers who perform functions on our behalf under written agreements limiting their use of the information.
With regulators, licensing bodies, accrediting organisations and law enforcement where required or permitted by law.

Protected health information and HIPAA
Medical information about our patients is protected health information governed by HIPAA and California law. Our separate Notice of Privacy Practices describes those protections and your rights in detail, and is provided to every patient at the start of care. Your California privacy rights
If you are a California resident you may request to know what personal information we have collected about you, the categories of sources, the purposes for collecting it, and the categories of third parties with whom we share it.
You may request deletion or correction of personal information, subject to exceptions for information we are required to retain, including medical and billing records.
You may opt out of the sharing of personal information for cross-context behavioural advertising.
You will not be discriminated against for exercising any of these rights.
